Field Trip Forms

Before going on a field trip through the high school, all staff members should make sure the following forms are filled out, turned in, and approved by administration. All staff should be aware of funding, budgeting, and grants that might be able to help fund field trips.

• SU76 Field Trip Request Form

• Field Trip Permission Slip Form

• Employee Leave Form

• Van Request Form (If Needed)

• Overnight Nurse Form

If a teacher needs a bus for their field trip, they must indicate that on the SU76 Field Trip Request Form and contact Andrea Brown at Browns Busing.
